چکیده

BACKGROUND We aimed to quantitatively assess the effect of baseline breast density on the incidence, stage, and mortality, and also the natural course of the disease, considering the sensitivity of mammography to clarify its causal or masking effect. METHODS In total, 15,658 women ages 45 to 59 years from the Kopparberg randomized controlled trial in Sweden were prospectively followed from 1977 until 2004 to ascertain breast cancer incidence and death. Dense breast tissue collected at the beginning of the study was defined as pattern IV or V by the Tabár classification. Conventional risk factors were also collected at baseline. The three-state Markov model was used to estimate the preclinical incidence rate and the mean sojourn time given the fixed sensitivity. RESULTS Dense breast tissue was significantly associated with breast cancer incidence [relative risk (RR)=1.57 (1.18-1.67)] and with breast cancer mortality [RR=1.91 (1.26-2.91)] after adjusting for other risk factors. Cumulative incidence rates irrespective of nonadvanced and advanced breast cancer were higher in dense breast tissue compared with nondense tissue but no difference in survival was detected between dense and nondense breast tissue. Dense breast tissue had a higher preclinical incidence rate (causal effect) and shorter mean sojourn time (masking effect) compared with nondense breast tissue by controlling the sensitivity of mammography. CONCLUSION We corroborated the effect of baseline breast density with a higher incidence and mortality and also showed its contribution to a masking effect with long-term follow-up data. IMPACT Results suggest that the screening policy with a predominantly shorter screening interval and with alternative imaging techniques might be indicated in women with dense breast tissue.

Swedish two-county trial: impact of mammographic screening on breast cancer mortality during 3 decades.

PURPOSE To estimate the long-term (29-year) effect of mammographic screening on breast cancer mortality in terms of both relative and absolute effects. MATERIALS AND METHODS This study was carried out under the auspices of the Swedish National Board of Health and Welfare. The board determined that, because randomization was at a community level and was to invitation to screening, informed ver...

Mammographic screening in women 40-49 years old.

The societal value of mammographic screening in women 40-49 years old is a controversial topic that is subject as much to emotional rhetoric as to scientific insight. This controversy is reflected in part in differences in screening guidelines developed by professional, voluntary, and government organizations. Early results from epidemiologic studies did not demonstrate conclusively that mammog...

Effect of mammographic screening from age 40 years on breast cancer mortality in the UK Age trial at 17 years' follow-up: a randomised controlled trial.

BACKGROUND Age-specific effects of mammographic screening, and the timing of such effects, are a matter of debate. The results of the UK Age trial, which compared the effect of invitation to annual mammographic screening from age 40 years with commencement of screening at age 50 years on breast cancer mortality, have been reported at 10 years of follow-up and showed no significant difference in...

The Swedish two county trial of mammographic screening for breast cancer: recent results and calculation of benefit.

The Swedish two county trial of breast cancer screening is now in its tenth year. This paper presents detailed results on mortality from breast cancer and from all other causes, and on the population denominators at risk for each of the first 8 years of follow up, for each county separately. These data represent a two year update on the last major report. Results show an increasingly significan...

A rationale for starting breast cancer screening under age 50.

Most countries which have implemented service screening on breast cancer invite women from age 50 [1]. With regard to younger women, findings from the UK age trial [2] showed the same trend as previous studies [3] that screening below age 50 has a positive effect on breast cancer mortality. For women screened at 40–49, Moss et al. [2] reported a breast cancer mortality reduction of 17% at 10-ye...
